What to Have Ready

Having the right information ready helps Dr. Adams review your health history and avoid delays while your prior records are being sent.

Records Release Form

Complete the authorization form so your previous provider can send records securely.

Medication List

Bring current prescriptions, over-the-counter medications, supplements, allergies, and dosages.

Recent Results

Bring recent labs, imaging reports, discharge paperwork, or specialist notes if you already have copies.

Insurance & Photo ID

Bring your current insurance card, photo ID, and any referral or authorization information your plan requires.

How to Transfer Your Care

Contact the Office

Call or send a general message to let the office know you are transferring care or establishing care with Dr. Adams.

Complete Authorization

Fill out the records release authorization with your identifying information and prior provider details.

Request Prior Records

Submit the completed form to your previous doctor, hospital, or specialist group and ask them to send records securely.

Prepare Key Information

Gather medications, allergies, insurance, recent labs, hospital paperwork, and specialist notes you already have.

Request Your First Visit

Once the records request is underway, request an appointment so Dr. Adams can review your history and next steps.
